What the form covers

One document, three parts: our terms and conditions, the medical disclaimer, and the liability release and assumption of risk agreement. It is the same form you would otherwise fill in on paper at the centre, and completing it online saves that time on your dive morning.

For the dive centre — issue a numbered form link

Forms are numbered from your own register, continuing the paper sequence: OL001 is the signed paper original, so the first form you issue online is OL002. Enter the booking reference and the next number, and this builds the link to send by WhatsApp, phone follow-up or email.

Terms and Conditions

1.All clients participating in SCUBA activities must sign the terms and conditions, liability release, assumption of risk and indemnity agreement, and complete a medical disclaimer.

2.All scuba activity participants, except those in beginner courses, must provide a valid certification from an internationally recognized agency.

3.Participation in scuba activities is at the individual's own risk.

4.Those under the influence of alcohol or drugs will be banned from activities without a refund.

5.The minimum age for scuba is 8 years for confined water and 10 years for water activities. Participants under 18 need parental or guardian approval, including signatures on medical statements, liability releases, and terms and conditions.

6.Scuba Duka regularly checks and maintains rental equipment, but participants are responsible for ensuring its proper working condition before diving. Use of rental equipment is at the participant's risk, and they will be charged for any negligent loss or damage to company property.

7.Scuba Duka accepts cash payments in Shillings, US$, Euros, British Pounds, and international major currencies. VISA and MasterCard are accepted forms of payment charged in US$ and incur additional 3% processing fee. Current market exchange rates apply.

8.All Scuba Duka activities require advance payment. No refunds are issued if the client doesn't use the service after the necessary infrastructure has been provided.

9.The participant agrees to follow Scuba Duka staff instructions, adhere to diving safety procedures, maintain a minimum of 50 bar pressure at dive's end, and act responsibly to protect the underwater environment.

Medical Disclaimer

Scuba diving is a physically demanding activity that carries inherent risks, including serious injury or death, and requires a specific level of physical and mental fitness. Our staff at Scuba Duka are not medical professionals and cannot provide medical advice; therefore, if you have any health concerns, you must consult a physician knowledgeable in dive medicine before participating. By engaging in diving activities, you acknowledge these risks and assume full responsibility for any consequences, including those arising from undisclosed or inaccurately reported health conditions. It is crucial that you disclose any past or present medical issues that might hinder or affect your diving safety.

I acknowledge that I fully understand the medical disclaimer, and I confirm that the information I have provided regarding my medical history is accurate to the best of my knowledge. I accept responsibility for any omissions related to my failure to disclose any existing or past health conditions.

Liability Release and Assumption of Risk Agreement

Please read carefully and fill in all blanks before signing.

I acknowledge the inherent risks of scuba diving, including serious injury or death, and understand that diving with compressed air carries specific dangers, such as decompression sickness. Despite these risks, I choose to proceed with instructional dives, assuming all associated risks and releasing the program and its parties from liability for any claims related to my participation. I am aware of the physical demands of scuba diving and the potential for various injuries. I confirm I am legally competent to sign this liability release and agree to the terms, waiving my rights to legal action. If any part of this agreement is unenforceable, the rest remains valid.
